Your feedback needed! Take the 2024 Community Health Needs Assessment

On October 1, 2024, the Community Health Needs Assessment (CHNA) survey was delivered to more than 4,000 Olmsted County households. This survey helps determine the greatest health needs of the county and find ways to improve residents’ health.
Olmsted County Public Health Services (OCPHS) asks those who receive the survey to fill it out and return it by mid-November 2024. Reminder postcards will be mailed to residents who do not return the surveys.
"We use survey information to identify what health issues are most important in our community," said OCPHS Director Denise Daniels. "Your answers, along with community feedback and local population health data, help us focus on the right areas to make the biggest impact.”
The survey takes about 20 minutes to finish and asks about your health care, environment, personal habits, and other things that affect your health. Your answers are completely private.
The survey is created by the Olmsted County Community Health Assessment and Planning (CHAP) team. CHAP started in 2012 and is led by OCPHS, Olmsted Medical Center, and Mayo Clinic. Many community organizations also coordinate with this effort to keep our community healthy.
After each CHNA survey, the CHAP team develops plans to address the top health issues. Below are highlights of work since the last survey in 2022.
- OCPHS is working with partners to improve care options for LGBTQ+ people.
- Zumbro Valley Health Center is providing more dental care for clients.
- United Way of Olmsted County is spreading the word about its 211 information and referral program, which connects people to local resources, 24/7 and in over 100 languages.
- Rochester Public Schools is piloting a mental health screening program in eight schools.
By participating in the 2024 CHNA survey, residents can directly influence health initiatives to shape the future of our community. Resident input ensures that resources and programs are targeted where they are needed most.
