Image consent form

From time-to-time, Olmsted County government employees capture photos, videos, and interviews on camera with county residents and visitors. These images are then used in various county communications materials and on different mediums.

To enable this process, we ask anyone featured on camera to complete an image consent form. By signing this form, the person whose image is being captured has authorized approval for the county to use that image in communications.

Resident/visitor (non-county employee) consent form
This form is applicable for Olmsted County residents, visitors, contractors, unpaid interns, and volunteers. Please use the above form if you authorize the use of your image in Olmsted County communications materials.

Resident/visitor (non-county employee) revocation form
This form is applicable for Olmsted County residents, visitors, contractors, unpaid interns, and volunteers. Please use the form above if you no longer wish to have your image used in Olmsted County communications materials.

For Olmsted County government employees only
(Olmsted County pays your wages directly)
